We welcome new patients who live at an address within the practice catchment area.
The practice catchment area is bordered by the following roads:
Romford Road, Upton Lane, Plashet Road, Plashet Grove and Shrewsbury Road.

How To Register
Non-urgent advice:
If you would like to register please complete the online registration form below and if you need help with registration please phone the surgery on Tuesday morning between 08.00 and 12.00.
Please take care to complete the form in correctly with all your details to allow us to register you easily.
We will then aim to contact you within five working days to arrange an appointment for your New Patient Registration Check with our practice nurse.
Please note we may request that your share with us your proof of address (such as a recent utility bill or bank statement) and proof of ID and also your medical card if you have one.
For children please bring the Red Book or other immunisation records if you have one.
If you do not have any of these items this does not prevent you from registering with the practice – please speak to a member of reception staff for further information.
